Observed chemical sanitizer being used on equipment and utensils in the wrong sequence. Instead of “wash, rinse, sanitize,” the sanitzer was being used in the 2nd compartment of the 3-compartment sink.

Non-food contact surfaces throughout the facility are not cleaned at a frequency to preclude accumulation of dirt and soil; specifically, the condensor fan covers inside a reach-in freezer, the floor of the inside of a reach-in freezer, walls, doors, light switches, shelves under prep tables and service areas and a tray used to hold clean coffee mugs.

Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use.

The food facility does not have a handwash sink that allows convenient use by employees in the food dispensing area.

5 – 205.11 Using a Handwashing Sink-Operation and Maintenance
A hose used for spraying equipment was connected to the faucet of the handwashing sink, indicating uses other than handwashing and making the handwash sink inaccessible for washing hands.

Exterior door located in the area of the food facility has a small gap and does not protect against the entry of insects, rodents, and other animals.

Facility is to take steps that will prevent the contamination of food; the storage shelving to be used for sushi ingredients and supplies is not appropriate for the amount and number of items stored in three locations – the walk-in cooler, the walk-in freezer and dry ingredient storage area.
