Theresa Murphy Kozlusky, RN, at age 78 and formerly of Port Carbon, PA passed into eternal rest on August 22, 2020 in the comfort of her home in Lebanon, PA.

Theresa was born in Glen Dower, PA, the daughter of Thomas and Marguerite Murphy. As a child, Theresa attended St. Kearins Elementary School. She graduated from Nativity BVM High School and earned her nursing degree in 1964 at St. Joseph’s School of Nursing in Reading.

For 56 years she has been married to Raphael R. Kozlusky, and together they raised three daughters. For 20 years, as a working mom, she was committed to her patients and staff at the former Rest Haven Nursing Home in Schuylkill Haven. Here her coworkers affectionately referred to her as “mother.” Prior to retirement, she held the position of Director of Nursing. Theresa and Raphael spent their post-retirement years in Lake Wales Florida at the Saddlebag Resort where they were surrounded by friends and family. Her last years were spent in Lebanon, PA where she was close to family and doctors who treated her multiple myeloma. Theresa and Ray attended St. Casimir’s Church in St. Clair, and are current members of Saint Paul the Apostle’s Parish in Annville.

In addition to her husband, she is survived by daughter Theresa Kozlusky Haley and husband Patrick Haley, with their children Mary Theresa, Daniel Thomas, and Dr. Katelin Haley-Yeich and her husband Douglas Yeich, parents of Haley and Adeline Yeich; daughter Janice Kozlusky Forest and husband Kenneth Forest, with their children Eric, Emily and Elizabeth; daughter Tina Kozlusky, with her son Maximos Ilsley and extended family Mike Ilsley and Rene Narkiewicz; sisters Eileen Secoges, Ann Smolsky, Gerry Maley (husband John), Shelly Symons (husband Tom); brothers James Murphy (wife Betty), Michael Murphy (wife Darlene); and many nieces, nephews, and their children. She was preceded in death by her parents, sisters Marguerite McTear and Patty Consugar, and brother Gerald Murphy.

Mass of Christian Burial will be celebrated on Wednesday, August 26th at 12:30 p.m. at St. Paul the Apostle Church, 125 S. Spruce St., Annville, PA 17003. Visitation will be held from 11:15 a.m. to 12:15 p.m. at Kreamer Funeral Home, 618 E. Main St., Annville, PA 17003. While attending services, please wear a mask and practice social distancing. Due to restrictions at the National Cemetery, interment will be private at Indiantown Gap.
